Steps to the complete MHS Student Athletic Clearance:
- Parent/Guardian must create an account to register student @ SportsNet
- Scroll to the bottom of the page and select "Create an Account"
- Complete ALL portions of the online profile
- Upload the medical eligibility form (please see form below)
