9th Grade Enrollment
NOW ENROLLING INCOMING 9TH GRADERS...
from outside the MPUSD school district which includes students coming from
Bay View Academy, Monterey Bay Charter, ISM, San Carlos, San Benancio, Big Sur Charter
Completed enrollment forms may be dropped off at the main office
M-F between 8 AM - 3 PM
Also send a copy or bring with you the following
- Acceptance letter if in Academy (MAOS or AMP)
- Transfer Approval Form, if the student is a non-resident of the school
- Name, address and FAX number of previous school
- Copy of Birth Certificate or Passport
- Immunization records (Proof of TDAP and all immunizations required to enroll in a school)
- Proof of your residence (lease/rental agreement of home address or utility bill *Gas, Water, Garbage dated within the last 30 days) Cell phone bills or car insurance statements are not accepted
**ONLY FILL OUT THE CAREGIVER AFFIDAVIT IF YOU ARE NOT THE PARENT AND ADDRESS VERIFICATION FORM IF THE UTILITY BILL IS NOT UNDER YOUR NAME
